Applications for permits may be obtained from any Hawaii Department of Agriculture Plant Quarantine Office or downloaded here in PDF format.

• The applicant must completely fill out the first page, including the quantity and scientific name of the organism desired, the object of importation and sign and date the bottom portion of the second page.
• For organisms on the Restricted Lists, please complete in detail the top portion of the second page, numbers 1 thru 5, which includes the reasons for introduction, persons responsible, safeguard facilities to be used, method of disposition, and an abstract of the organism.

There is a fee of:

1. $5.00 per permit for a single shipment within one year from date of issuance [fee only applies to the importation of animals (aquatic, land and insects) and microorganisms].
2. $50.00 per permit for unlimited shipments within one year from date of issuance [fee only applies to the importation of animals (aquatic, land and insects) and microorganisms].
3. A fee for a site inspection as required shall be $25.00 per inspection plus mileage reimbursement.
4. Fees may be waived for not-for-profit organizations, universities, government agencies, or for exhibition in municipal zoos or government-affiliated aquariums.
