HAWAII ANIMAL PREMISES IDENTIFICATION SYSTEM

The Hawaii State Department of Agriculture (HDOA) has adopted the federal standardized premise registration system, matching with the USDA/National Animal Identification System (NAIS), to identify animal premises in the State.  The goal of the Hawaii Animal Premises Identification System (HAPIS) is to rapidly trace animal origin for control/prevention of disease spread in an event of outbreak in the State. In order for animal health officials to trace animals, exact location of premises where animals are born, housed, or moved must be known. 

Premises registration is voluntary at this time, and the registration form is available on this web site or at the Animal Industry Division, 99-941 Halawa Valley Street, Aiea, Hawaii 96701 (Phone: 808-483-7100).  All information collected for the purpose of HAPIS is confidential and will be used only for an animal health purpose.

An exact postal address of your animal premises is required to register into the HAPIS. If you do not have postal address for the animal premise, please provide your Tax Map Key (TMK) number and an accurate description of your animal premise location.

To obtain a premises ID, you must provide all required information on the registration application and be registered in the HAPIS database.  Inaccurate or incomplete premise address will result in no Premise ID available from the system.
